Alevea AllCare Membership Terms and Conditions
These Terms and Conditions (“Agreement”) govern your participation in the Alevea AllCare Membership program, offered by Alevea Mental Health (“Alevea”, “we”, “us”, or “our”). By enrolling in and utilizing the Alevea AllCare Membership, you (“Member” or “Patient”) agree to be bound by these Terms and Conditions. Please read them carefully before participating in the program.
1. Membership Overview
The Alevea AllCare Membership is a subscription-based healthcare program that provides members with access to specific health services for a fixed monthly fee of $119. This membership includes access to psychiatric medication management, primary care, medical weight management, and hormone optimization services, with each visit priced at $30.
2. Services Included
The membership entitles you to:

Primary Care

Medical Weight Management

Hormone Optimization Each visit is $30 per appointment under the membership, which would otherwise cost $149 per cash pay visit.
Additional services such as therapy, laboratory work, diagnostic tests, and referrals are not included in the membership and are charged separately. Therapy is available at a discounted rate of $110 per session for members, reduced from the usual rate of $160 per session.
3. Eligibility
To be eligible for the Alevea AllCare Membership, you must:

Be at least 18 years of age.

Reside within the state of Arizona.

Agree to these Terms and Conditions and provide accurate personal and payment information.
4. Membership Fees and Payment Terms

The monthly membership fee is $119, payable through automatic credit card billing on the date you enroll and renews monthly on that date.

Each appointment under the membership is priced at $30 and will be charged separately from the monthly membership fee.

By enrolling, you authorize Alevea to charge the credit card on file for the monthly membership fee and the $30 per-visit fee.

Membership fees are subject to review and adjustment. Alevea reserves the right to modify the monthly fee and appointment fees with 30 days' written notice to the Member.
5. Cancellation and Refund Policy

You may cancel your Alevea AllCare Membership at any time by providing 30 days' written notice. To cancel, contact Alevea’s administration by email or phone.

Cancellations will take effect at the end of the current billing cycle following the 30-day notice. No refunds or partial refunds will be issued for any remaining days in the billing cycle.

If the membership is canceled, you may be responsible for any services rendered during the final billing period.

Alevea reserves the right to cancel the membership at any time if there is non-payment, abuse of services, or any violation of these Terms and Conditions.
6. Non-Covered Services

The membership fee covers only the costs of appointments for the services listed in Section 2. All other services, such as laboratory work, diagnostic tests, referrals, and medications, are billed separately and remain the financial responsibility of the Member.

Alevea does not upcharge or profit from the cost of these additional services, ensuring transparency.
7. Membership Renewal and Changes

The Alevea AllCare Membership will automatically renew each month unless canceled by the Member or Alevea in accordance with the Cancellation Policy.

Alevea reserves the right to amend, modify, or terminate the membership plan or any benefits associated with it. Any changes will be communicated to Members in writing at least 30 days prior to their effective date.
8. Medical Necessity and Provider Discretion

Alevea providers maintain the sole discretion to determine the medical necessity of appointments and treatments. Appointments and services must be deemed medically necessary by your provider.

Alevea reserves the right to adjust, reschedule, or cancel appointments if they are deemed unnecessary or inappropriate for the Member’s care plan.
9. Credit Card and Payment Information

A valid credit card is required to enroll in the Alevea AllCare Membership and to maintain the membership in good standing.

Should your credit card information change, you are required to update the payment method with Alevea promptly to avoid interruptions in membership services.

If a payment is declined, Alevea reserves the right to suspend services and the membership until payment is made in full.
10. Non-Payment and Suspension of Services

Non-payment of fees may result in the suspension or termination of membership. Alevea reserves the right to suspend or terminate services for any account more than 30 days past due.

If your membership is suspended due to non-payment, services will be resumed only after all outstanding balances are settled.
11. Confidentiality and Privacy

Alevea is committed to protecting your privacy and will handle all personal and medical information in accordance with HIPAA regulations.

Your personal and medical information will not be disclosed to third parties without your consent unless required by law or for billing purposes in accordance with our Privacy Policy.
12. Indemnification
By enrolling in the Alevea AllCare Membership, you agree to indemnify, defend, and hold harmless Alevea Mental Health, its officers, directors, employees, contractors, and agents from any claims, losses, damages, liabilities, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to:

Your breach of any provision of this Agreement.

Any services or treatments received under the membership plan.

Any claims related to or arising out of misuse, abuse, or over-utilization of the membership benefits.
13. Limitation of Liability
Alevea, its officers, directors, employees, contractors, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to the membership or the services provided under it.
In no event shall Alevea’s total liability to you for all claims, damages, losses, and causes of action exceed the amount paid by you to Alevea in the three (3) months preceding the date the claim arose.
14. Governing Law
These Terms and Conditions shall be governed by and construed in accordance with the laws of the state of Arizona, without regard to its conflict of law principles. Any dispute arising out of or related to these Terms and Conditions or the Alevea AllCare Membership shall be resolved exclusively in the courts of Maricopa County, Arizona.
