Medication Refill request

Please submit your Medication Refill Request with the form below. Once your request is submitted a staff  member will review and, if any questions, will contact you by phone. Please allow up to THREE business days for your request to be processed. 

We understand the importance of not running out of your medications and will work with you to ensure you have an adequate supply. Please continue with your scheduled appointments as patients with repeated No Shows or frequent Cancellations will not have their request completed and will need to schedule a follow up with the provider. 

We will not process any controlled medication request i.e. Adderall, Vyvanse, Concerta, Clonazepam, Suboxone (Buprenorphine). You must make a Follow Up appointment with your provider, no exceptions.
